These are the general layout of the forms inside. There is minor differences relating to cat or dog, generally to do with species.

Sire and Dam in each box.
Breed, Variety, Date of Birth, Breeder, Address, Registration no, Registration date, Owner. (depending whether cat or dog pedigree).

All you need to do is fill them with the required information.
